Fort Bragg's Home Alone Policy

One stop registration for all CYS patrons

• Full day and part day
• Waitlist for child care
• Babysitter referral
• Parent education classes

Necessary forms & records to register for CYS:

• Military ID card or proof of employment for DoD and Federal Government employees.
• Legal/official proof of child's birth (i.e. passport, ID card, birth certificate.)
• Legal/official proof of child's social security card.
• Current shot records for children birth-4th grade.  Shot records are not required for grades 5 and above.
• Local emergency points of contact, addresses and telephone numbers and one point of contact.
• Sponsor's and spouse's most current income statement.  Total household income will determine the fee category in child care programs.
• Legal guardianship papers to register children for childcare.

* Special Needs children require medical documentation prior to registering Child and Youth Services.  Allergies are also considered a special need.

Annual registration fee is $18 per child not to exceed $40 per family. CYS membership is required to purchase a pool pass for Tolson Youth Center. CYS replacement membership cards are $5 each.

Children that are not of school age will not have their pictures printed on their cards.  Children Kindergarten and above are required to have pictures printed on their cards.  Parents may complete the registration packet and bring child to registration office at a later date to have cards printed.
